Medicaid Application Instructions


You can apply for Alaska Medicaid in different ways. You can apply online, fill a paper form, or simply visit a local healthcare office for enrollment in Medicaid.

Apply online

To apply for Medicaid online, you can to visit my.alaska.gov where you can complete an online enrollment application.

Alternatively, you can apply online for Medicaid through Healthcare.gov.

The online application is the fastest way to enroll in Medicaid in Alaska. However, if you don't have access to the internet, you can apply using any of the following methods as well.

Apply in person

You can apply for Alaska Medicaid by visiting your nearest local office in person. You can use this tool to find your nearest office for Medicaid. Alternatively, you can call a local office to learn about enrollment. Visit this page to find a contact number for Medicaid offices in Alaska.

Apply through mail

You can also apply for Medicaid using mail or fax. To do so, you will need to download and fill a paper application. You will then mail or fax it to your local Medicaid office. To download a paper application, visit this link.


Eligibility Requirements

To apply for Medicaid, you will need to prove your eligibility. The program provides benefits to the residents of Alaska who fall into one or more of the following Medicaid categories.

-Parents and caretakers

-Children under the age of 19 years

-Young adults (under 21 years of age)

-Former foster care children (age limit is 26 years)

-Pregnant women

Income limits for the program

For children under the age of 19 years, the household income must be less than 203 percent of the Federal Poverty Level (FPL).

For pregnant women, the income limit is set at 200 percent of the FPL.

For parents and caretakers, the income limit for Medicaid is set at 129 percent of the FPL.

The FPL guidelines for Medicaid in Alaska are provided below.

-$14,720 for a household consisting of 1 person.

-$19,920 for a household consisting of 2 members.

-$25,120 for a household consisting of 3 members.

-$30,320 for a household consisting of 4 members.

-$35,520 for a household consisting of 5 members.

-$40,720 for a household consisting of 6 members.

-$45,920 for a household consisting of 7 members.

-$51,120 for a household consisting of 8 members.

For households with more than eight members, add $5,200 for each additional member to calculate FPL.
