Apply in Person
You can apply for SCHIP in person by visiting your local Health and Welfare office. Once you are at the office, you will be helped with rest of the things. You will be able to obtain an application from the office. If you need to ask any questions about your child's health insurance, you will be able to get help from the office. You can call Idaho CareLine and ask for an application to be mailed to you. Call at 1 800 926 2588 or 2 1 1 to request application. You can either return the completed application by mail or drop it in person at your nearest Health and Welfare office.
What you need to know before you apply
When you apply for SCHIP, you will be required to provide a proof of citizenship along with identity and social security number. You will also have to provide residency documents and information about any health insurance your child had in the past. You will be required to provide a proof of your income and income tax payments.
You will have to submit all these documents with the application form.
If you are receiving benefits for your child under SCHIP in Idaho and your circumstances (financial and non-financial) have changed, you're responsible to report them to related office at the earliest. These changes may include change in income, change of phone number, change in social security number, or even a newly diagnosed disability.
In order to qualify, your child must be:
- A U.S citizen or a legal alien
- Resident of Idaho
- Under the age of 19 years
- Child should not have a private medical insurance
- Family income is high for the purpose of Medicaid but it should be within SCHIP limits. You can see the complete chart sorted by size of household. Families with monthly income below 185% of federal poverty level will be able to meet financial criteria.
