WIC Application Instructions


To apply for Indiana WIC, you will have to call your nearest WIC clinic. You can find contact information for your nearest WIC clinic using the map available by clicking here.

When you call and get an appointment, make sure you ask what documents to take with you. Usually, you will have to take your income proof, residence proof, and identity proof with you.


Eligibility Requirements

Indiana WIC requires applicants to meet certain eligibility criteria before they can start receiving WIC benefits. The program only targets:

  • Women (pregnant, postpartum, and breastfeeding)
  • Infants up to age of 1 year
  • Children up to age of 5 years

Applicants falling to the above-mentioned categories will have to meet following requirements

  • Must be living in Indiana
  • Have household income within income guidelines
  • Facing nutrition risk (medical or health risk)

In order to meet income eligibility, applicants will have to have their household income (before deductions) within the following limits

Those already receiving Medicaid, TANF or SNAP benefits will automatically qualify for Indiana WIC.
