LIHEAP Application Instructions


In order to apply for Utah LIHEAP - HEAT, you will need to download appropriate application for your household from the links below.

Once you have downloaded appropriate application, you will need to complete it in all respects. You will also need to attach copies of various documents that will be used to verify your credentials and income. Don't attach originals as they will not be returned.

Before mailing your application, make sure that it is complete and contains all the necessary documents. Incomplete applications will be rejected.

Eligibility Requirements


In order to receive benefits through Utah HEAT - LIHEAP, applicants will need to meet certain eligibility criteria for the program:

Citizenship: Applicant must be a U.S citizen (naturalized or U.S born) or a qualified alien. A person will be considered a qualified alien if he/she has been documented by the U.S Citizenship and Immigration Services. Temporary entrants such as visitors, students, tourists, and diplomats are not eligible for the Utah HEAT - LIHEAP benefits.

Residence: You must be a resident of state of Utah to receive LIHEAP - HEAT benefits. The members of Utah's five Native American tribes can also apply for assistance just like other residents. However, they can also apply through local Tribal LIHEAP if it's available. If a household applies through both offices, it will receive assistance from one office only. Your local HEAT office will verify whether or not you have received assistance from Tribal LIHEAP before issuing benefits.

Responsible for paying home heating bills: Applicant must be responsible for paying household heating bills in order to qualify for the assistance. Those paying heating bills indirectly, as in case of heating cost being a part of rent; can also be considered eligible for receiving LIHEAP - HEAT assistance if they meet other requirements for the program.

Income eligibility: In order to receive benefits through LIHEAP - HEAT, a household must meet income guidelines.
