WAP Application Instructions

To apply for Utah WAP, you will just need to go to your nearest WAP provider. WAP services are provided through eight government and non-profit agencies. These agencies cover each county so that needy households can find a WAP provider in their county.

These WAP providers will help you with application process. You can obtain the application bundle from the office and hand it back to the office after you’ve completed it. As soon as you return your completed application form, you will be added to WAP list if you meet qualification criteria for the program.

Alternatively, you can download application form from the internet and fax or mail completed application with necessary documents to your county WAP provider. You can also drop it off at the office in person instead of mailing or faxing it to your provider. Download the application form here.
