WIC Application Instructions
To apply for Iowa WIC program, you will have to make an appointment with your local WIC program. You can make an appointment through phone or by visiting the office in person. You can find phone number of WIC program near you using this link: iowa.maps.arcgis.com
On your first visit to the office, you will need to make sure that you are carrying your proof of Iowa residence, proof of identity, and proof of income with you. The office will determine your eligibility after watching your documents.
Eligibility Requirements
In order to receive WIC program benefits, one has to meet program eligibility criteria. The following persons are eligible for WIC program:
- Infants and children up to five years of age
- Pregnant women
- Breastfeeding women
- Postpartum women
For all the people in these categories, they also have to be considered a nutritional risk as determined by a health professional.

Automatic Income Eligibility
For households where one or more members receive SNAP, Medicaid, TANF, or benefits from certain other programs are automatically income eligible for Iowa WIC program.
